Conditions and Declaration

In signing and submitting this ‘Enrolment Form’ and this declaration section the student and parent or guardian (where applicable) is entering into a contract with MSA Training and Professional Development and agrees that:

  • Once the student commences the nominated course, MSA Training and Professional Development will deliver the Training Program using competency based training principles and practices in accordance with the AQTF (Australian Quality Training Framework)
  • MSA Training and Professional Development and the student will work together to produce a unified approach in the student achieving the relevant qualification.
  • Where a student has undertaken an assessment and it has been marked as ‘Not Yet Competent’ (NYC), they be allowed to re-sit the test/or have a re-assessment. If they are deemed ‘NYC’ for a second time they are to re- enrol into that unit/subject. This will include re-training and therefore a re-enrolment fee for the unit will apply.
  • For re- issuance of Certificates and Statements of Attainments the student will be charged $22 per document required. Payment for the re-issuance of such documents is required prior to the re-issuance occurring.
  • MSA Training and Professional Development reserves the right to accept or reject any application for enrolment at its discretion.
  • MSA Training and Professional Development reserves the right to cancel any course prior to the commencement date of the course should it deem it necessary and in that event, shall refund all payments received from the student (see Refund Policy).
  • The information provided by the student in this enrolment form is complete and correct and this information will be retained by the RTO (Registered Training Provider) for the purposes of enrolment and/or government funding.
  • Providing any false information and / or failing to disclose any information relevant to my application for enrolment and / or failure to complete an application / enrolment form may result in the withdrawal of any offer, particularly as it relates to my eligibility to obtain an offer for government subsidised training, and / or cancellation of enrolment at the discretion of MSA Training and Professional Development.
  • To be bound by the MSA Training and Professional Development rules and regulations and any amendments made to the rules and regulations.
  • Language, Literacy and Numeracy testing is a requirement prior to acceptance into any course entry and adhere to any other pre-requisites identified through the enrolment process.
  • Where a student’s contact details change while studying with MSA Training and Professional Development the student must advise MSA Training and Professional Development of these changes within 7 days. These details include but are not limited to details such as e-mail, address, and contact phone details.
  • The Student Information Handbook has been read and understood.
  • The Refund Policy has been read and understood.
  • Payment is attached for enrolment into this course as outlined in this enrolment form has been made for the course. Default of a payment plan may result in my enrolment being suspended or withdrawn.
  • Once your course has commenced the fees must be paid in full.
  • Course fees are not able to be changed after your course has commenced.
  • If you become eligible, or are no longer eligible, for a concession part-way through your course, your fees will remain the same amount as when you enrolled.
  • I have read the eligibility criteria for the Skills Victoria Government Funding.
  • Enrolment in a government funded place may impact on your ability to access future government funding.
  • If you have an exemption to the requirement for a USI then the results of your training will not be accessible through the Commonwealth and will not appear on any authenticated VET transcript prepared by the USI Registrar.
  • I am aware that, if I am enrolled into a classroom based course, it may be moved online onto Zoom should Covid restrictions be announced.
  • The MSA Executive Officer is responsible to ensure that there are suitable arrangements to provide records of student outcomes to ASQA in the event that MSA Training and Professional Development ceases to operate. This will be provided via an AVETMISS report encompassing all activity data for the duration of MSATPD operation. If circumstances are such that MSATPD are not able to carry on operations, the RTO Director is to advise ASQA immediately of this decision and provide advice regarding impact on currently enrolled students. MSATPD has a responsibility to transfer these enrolments to another RTO with least disruption to individual students. On ceasing operations, MSATPD is to, facilitate currently enrolled students transfer to another RTO, issue these students with an appropriate refund for service not provided, and issue students with a Statement of Attainment based on completed units of competency.

National VET Data Policy

Purpose

This policy explains how National Centre for Vocational Education Research Limited (NCVER) staff and contractors manage your personal information.

NCVER is committed to managing personal information in an open and transparent way. Our Privacy Policy explains how the NCVER complies with:

  • the Privacy Act 1988 (Privacy Act), including the Australian Privacy Principles (APPs)
  • Subdivision B of the National Vocational Education and Training Regulator Act 2011 (NVETR Act)
  • Divisions 4 and 5 of the Student Identifiers Act 2014 (SI Act) and the Student Identifiers Regulations
  • the National Vocational Education and Training Regulator (Data Provision Requirements) Instrument 2020
  • the National VET Data Policy.

We only collect the information we need to do our work. We regularly review our processes to make sure the information we collect is protected in the best possible ways.

Why we collect your personal information

As a registered training organisation (RTO), we collect your personal information so we can process and manage your enrolment in a vocational education and training (VET) course with us.

Collection of your data

Monash Student Association is required to provide the Department with student and training activity data. This includes personal information collected in the Monash Student Association enrolment form and unique identifiers such as the Victorian Student Number (VSN) and the Commonwealth’s Unique Student Identifier (USI). I consent to my identity documents being disclosed to the government’s Document Verification Service

Monash Student Association provides data to the Department in accordance with the Victorian VET Student Statistical  Collection  Guidelines, available at:

How we use your personal information

We use your personal information to enable us to deliver VET courses to you, and otherwise, as needed, to comply with our obligations as an RTO.

How we disclose your personal information

We are required by law (under the National Vocational Education and Training Regulator Act 2011 (Cth) (NVETR Act)) to disclose the personal information we collect about you to the National VET Data Collection kept by the National Centre for Vocational Education Research Ltd (NCVER). The NCVER is responsible for collecting, managing, analysing and communicating research and statistics about the Australian VET sector.

We are also authorised by law (under the NVETR Act) to disclose your personal information to the relevant state or territory training authority.

How the NCVER and other bodies handle your personal information

The NCVER will collect, hold, use and disclose your personal information in accordance with the law, including the Privacy Act 1988 (Cth) (Privacy Act) and the NVETR Act. Your personal information may be used and disclosed by NCVER for purposes that include populating authenticated VET transcripts;
administration of VET; facilitation of statistics and research relating to education, including surveys and data linkage; and understanding the VET market.

The NCVER is authorised to disclose information to the Australian Government Department of Education, Skills and Employment (DESE), Commonwealth authorities, State and Territory authorities (other than registered training organisations) that deal with matters relating to VET and VET regulators for the purposes of those bodies, including to enable

  • administration of VET, including program administration, regulation, monitoring and evaluation
  • facilitation of statistics and research relating to education, including surveys and data linkage
  • understanding how the VET market operates, for policy, workforce planning and consumer information.

The NCVER may also disclose personal information to persons engaged by NCVER to conduct research on NCVER’s behalf.

The NCVER does not intend to disclose your personal information to any overseas recipients.

Surveys

You may receive a student survey which may be run by a government department or an NCVER employee, agent, third-party contractor or another authorised agency. Please note you may opt out of the survey at the time of being contacted.

Consequences of not providing your information

Failure to provide your personal information may mean that it is not possible for you to enrol in VET and/or to obtain a Victorian Government VET subsidy.

Access, correction and complaints

You have the right to seek access to or correction of your own personal information. You may also complain if you believe your privacy has been breached.

Declaration

I hereby consent to the disclosure of my personal information to PharmacyID. I authorise them to use this information for the purposes of verifying my identity, assessing my eligibility for government funding and determining the authenticity of the documentation I supply as part of my enrolment. This includes documentation such as my medicare card, drivers licence, passport, Concession Card, birth certificate or immigration documents.

The Victorian Government, through the Department of Education and Training (the Department), develops, monitors and funds vocational education and training (VET) in Victoria. The Victorian Government is committed to ensuring that Victorians have access to
appropriate and relevant VET services. Any personal information collected by the Department for VET purposes is protected in accordance with the Privacy and Data Protection Act 2014 (Vic) and the Health Records Act 2001 (Vic).

Collection of your data
Builders Academy Australia (BAA) is required to provide the Department with student and training activity data. This includes personal information collected in the Builders Academy Australia (BAA) enrolment form and unique identifiers such as the Victorian Student Number (VSN) and the Commonwealth’s Unique Student Identifier (USI).
Builders Academy Australia (BAA) provides data to the Department in accordance with the Victorian VET Student Statistical Collection Guidelines, available at DET website.

Use of your data
The Department uses student and training data, including personal information, for a range of VET purposes including administration, monitoring and planning, including interaction between the Department and Student where appropriate.
The data may also be subjected to data analytics, which seek to determine the likelihood of certain events occurring (such as program or subject completion), which may be relevant to the services provided to the student.

Disclosure of your data
As necessary and where lawful, the Department may disclose VET data, including personal information, to its contractors, other government agencies, professional bodies and/or other organisations for VET-related purposes. In particular, this includes disclosure of VET student and training data to the Commonwealth and the National Centre for Vocational Education Research (NCVER).

Legal and Regulatory
The Department’s collection and handling of enrolment data and VSNs is authorised under the Education and Training Reform Act 2006 (Vic). The Department is also authorised to collect and handle USIs in accordance with the Student Identifiers Act 2014 (Cth) and the student Identifiers Regulation 2014 (Cth).

Survey participation
You may be contacted to participate in a survey conducted by NCVER or a Department endorsed project, audit or review relating to your training. This provides valuable feedback on the delivery of VET programs in Victoria. Please note you may opt out of the NCVER survey at the time of being contacted.

Consequences of not providing your information
Failure to provide your personal information may mean that it is not possible for you to enrol in VET and/or to obtain a Victorian Government VET subsidy.

If you do not already have a Unique Student Identifier (USI) and you want MSA Training and Professional Development to apply for a USI to the Student Identifiers Registrar (Registrar) on your behalf, MSA Training and Professional Development will provide to the Registrar the following items of personal information about you:

  • Your name, including first or given name(s), middle name(s) and surname or family name as they appear in an identification document;
  • Your date of birth, as it appears, if shown, in the chosen document of identity;
  • Your city or town of birth;
  • Your country of birth;
  • Your gender; and
  • Your contact details.

I am providing one of the following forms of ID for the purposes of creating a USI: Australian passport

  • Non-Australian passport with Australian visa Australian birth certificate
  • Australian driver’s licence Medicare card
  • Certificate of registration by descent Citizenship certificate
  • Immicard

Privacy Statement

I understand that:

MSA Training and Professional Development is required to provide the Victorian Government, through Skills Victoria, with student and training activity data which may include information I provide in this enrolment form. Information is required to be provided in accordance with the Victorian VET Student Statistical Collection Guidelines (which are available at www.skills.vic.gov.au/corporate/statistics/submit_data). Skills Victoria may use the information provided to it for planning, administration, policy development, program evaluation, resource allocation, reporting and/or research activities. For these and other lawful purposes, Skills Victoria may also disclose information to its consultants, advisers, other government agencies,

professional bodies and/or other organisations.

The Education and Training Reform Act 2006 requires MSA Training and Professional Development to collect and disclose my personal information for a number of purposes including the allocation to me of a Victorian Student Number and updating my personal information on the Victorian Student Register.
